***Medications and doctor's orders must be renewed EVERY school year.
***Immunizations must be kept current. Report all new immunizations to the school nurse.

If your child has ASTHMA - Please print and complete and return the Physician's Medical Report, Medication Administration Form, and Asthma Action Plan.
If your child has SEIZURES - Please complete and return the Physician's Medical Report, Medication Administration Form, and Seizure Action Plan.
If your child has FOOD ALLERGIES - Please complete and return the Physician's Medical Report, Medication Administration Form, and Food Allergy Action Plan.
If your child has ANY OTHER MEDICAL DIAGNOSIS THAT REQUIRES MEDICATIONS - Please complete and return the Physician's Medical Report, Medication Administration Form, and Food Allergy Action Plan.
